Two switches, not one: sleep pressure vs arousal

Feeling exhausted but unable to sleep is not a paradox once you separate the two systems that govern sleep. Homeostatic sleep pressure, driven by adenosine building up the longer you are awake, is what makes you feel tired. A separate arousal system, the fight-or-flight wiring driven by cortisol and the sympathetic nervous system, can override that tiredness and keep you alert. The hyperarousal engine

Researchers describe chronic insomnia as a disorder of hyperarousal (Bonnet & Arand 2010): people who cannot sleep despite being tired tend to show elevated cortisol in the evening, faster heart rate, higher body temperature, and more high-frequency EEG activity at sleep onset. In other words the body is running in a mildly activated state when it should be powering down. Acute stress, a demanding day, worry, and even the anxiety of not sleeping all feed this engine, which is why the harder you try to force sleep the more wired you can become. The practical target is de-arousal, deliberately lowering that activation before and in bed.

Late caffeine and alcohol: hidden wirers

Two everyday inputs keep the arousal system lit. Caffeine blocks adenosine, the very molecule that signals tiredness, and it has a long half-life: Drake et al. 2013 found caffeine taken even six hours before bed measurably disrupted sleep, so an afternoon coffee can leave you tired-but-wired at night without your noticing the link. Alcohol is the opposite trap, it sedates at first but then fragments the second half of the night and can produce a wired 3am wake as it clears. Bright evenings and the delayed clock

Sometimes the wired feeling is your body clock, not just stress. Bright light in the evening suppresses melatonin and pushes the circadian phase later (Gooley et al. 2011; Chang et al. 2015), so your biological night has not started when your alarm-dictated bedtime arrives: your body is still in its alert window. This is the classic late-chronotype or delayed-phase pattern, an evening 'second wind' followed by trouble falling asleep. Dimming and warming light in the last two to three hours lets melatonin rise on schedule. If the wired-at-night pattern is chronic, it may be a delayed clock rather than insomnia, and evening light hygiene plus a fixed wake time is the lever.

The wind-down that actually lowers arousal

Because the problem is arousal, the wind-down has to do more than fill time: it has to shift the nervous system toward parasympathetic. Give yourself 30 to 60 minutes of genuinely low-stimulation activity, then use an active de-arousal technique that has evidence behind it, slow breathing with a longer exhale, progressive muscle relaxation, or a non-sleep deep rest (NSDR) or body-scan practice. These lower heart rate and physiological tension rather than just distracting the mind. A cool, dark, quiet bedroom supports the same shift. The aim is to let the tiredness you already have take over once the arousal brake is released.

Do not fight it in bed: stimulus control

The worst response to being wired in bed is to lie there trying harder, because it teaches your brain to associate the bed with frustrated wakefulness and deepens the arousal. Keep a fixed wake time to anchor both sleep pressure and the clock, and if you are still wired after roughly 20 minutes, get up, keep the lights low, do something calm and boring, and return only when sleepy. This stimulus-control approach protects the bed-sleep link and is a core part of the structured treatment for insomnia. If tired-but-wired nights persist for weeks, that points toward chronic insomnia, where cognitive behavioral therapy for insomnia is the first-line answer.

Why am I so tired but cannot fall asleep?

Because tiredness and the ability to fall asleep are controlled by two different systems. Sleep pressure, built by adenosine while you are awake, makes you feel tired. A separate arousal system, driven by cortisol and the sympathetic nervous system, can stay switched on and override that tiredness, keeping you alert in bed. Not always, but it is the core experience of insomnia. An occasional wired night after a stressful day or too much afternoon coffee is normal. When the pattern of being exhausted yet unable to sleep happens at least three nights a week for three months or more and affects your days, it meets the definition of chronic insomnia, which is driven largely by hyperarousal. In that case a structured program (cognitive behavioral therapy for insomnia) is the recommended first-line treatment rather than long-term sleeping pills.

Yes. If you reliably get a 'second wind' in the evening and only feel sleepy very late, the issue may be a delayed circadian phase rather than stress-driven insomnia. Bright evening light suppresses melatonin and pushes the clock later, so your biological night has not begun at your intended bedtime. Dimming and warming light in the last two to three hours, holding a fixed wake time, and getting bright light soon after waking pull the clock earlier. If it is entrenched, it may be delayed sleep phase, which has its own protocol.
